 This is some sort of unnatural cross between a bull and a man. Quite different from the minotaur of legend, it possesses a shaggy white bull's head on an otherwise unremarkable human body. Clad in sagging socks and stained jockey shorts, it grunts and snuffles, drooling ropey strands of white slobber down its chest. Its mere presence fills you with an unfathomable dread. 

This creature is a human sized nether inhabitant. It's a competent fighter with insanely high hit points. It's slower than you, agile, has no armor against bashing weapons and no armor against cutting ones (that includes bullets). Its special ability allows it to paralyze you with its horrifying stare, every 20 turns.

It has extremely high morale and it's mostly aggressive. Being a nether inhabitant, it's afraid of being hurt, and can be angered by nothing in particular.

It can see, is able to smell characters, can hear noises, is warm blooded (it appears in infrared), can bash destroyable objects in its way (like doors), gets help from surrounding monsters when bashing objects, is considered an animal (it's affected by animal empathy), walks around some dangerous terrain and traps, can produce fur when butchered, doesn't breathe (it's not affected by smoke, toxic gases, drowning, etc), can produce bones and sinews when butchered, can produce fat when butchered, walks around some dangerous terrain and traps, and nothing else.

Being made of flesh, it's vulnerable to nothing in particular.

When it dies, nothing unusual happens.

Supposedly the "Cow Head" story is so horrifying that people who read or hear it are overcome with fear so great that they tremble violently for days on end until they die. Source
